PDIA Social Work Leadership Award

The Social Work in Hospice and Palliative Care Network will grant a Project on Death in America (PDIA) Social Work Leadership Award to a social worker who demonstrates outstanding leadership and has advanced the field in end-of-life, hospice and palliative care. The recipient will attend the annual conference of a selected social work or palliative care professional organization where they will receive this award and have an opportunity to present a summary of their work. The recipient will also be encouraged to publish their work in a professional journal. The goals of this very special professional award are to acknowledge and increase the visibility of social workers’ contributions to end-of-life, hospice and palliative care and encourage future generations to continue providing quality care to the seriously ill, dying and bereaved.

Description

This award will be given annually and will acknowledge the contributions of outstanding social work leaders in the specialized area of social work in palliative and end of life care. Nominations are sought for individuals who have made significant contributions to advancing social work practice in one or more of the following areas: research, education, practice, and policy. This unique award is created through the generous support and vision of the Project on Death in America.

Nomination Process: Eligibility

  • Must hold a degree in Social Work from an academic institution recognized by the Council on Social Work Education or a comparable accrediting body in the nominee’s country
  • Have exemplified excellence and leadership in end-of-life, hospice and palliative care that has reached beyond their immediate institution/place of employment

Application must include:

  • Two letters of support (not written by the nominator) which clearly demonstrate the nominee’s skills, talents and accomplishments related to hospice and palliative care, emailed to award@swhpn.org

The award will be presented at the annual meeting of an organization chosen by the Social Work in Hospice and Palliative Care Network Award Committee, working in collaboration with the awardee, for its relevance to the field and the awardee’s specific accomplishments. The awardee will be asked to either give a presentation and/or host a forum with conference attendees interested in speaking with an accomplished practitioner in end-of-life, hospice and palliative care social work.The recipient will also be encouraged to publish their work in a professional journal, such as the Journal of Palliative Medicine or the Journal of Social Work in End-of-Life and Palliative Care. In addition to the honor associated with selection by professional peers, the award includes a monetary grant of $1,000 and travel expenses to/from the conference.

There are two ways to submit an application for this award. Nomination forms can be submitted electronically and can be accessed at http://swhpn.org/nomination/. Though the online form is preferred, hard copy applications will be accepted and can be downloaded here: http://www.swhpn.org/nominations.pdf. Hard copy applications must be faxed with all required supporting documentation to: (215) 975-2100. Applications submitted electronically receive the same consideration as applications submitted by fax. Deadline

Completed nomination packets must be received no later than April 15, 2008. The individual selected to receive the 2008 Project on Death in America Social Work Leadership Award, and the individual who nominated them, will receive notification no later than May 30, 2008. .This award is made possible by a grant from the Open Society Institute.
